Brief description of the problem covered by this project:

The COVID-19 pandemic has made clear the need to transform care for vulnerable people affected by Alzheimer’s. Ace Alzheimer Center Barcelona wants to guarantee the best quality of care and follow-up to this group to minimize risks and give adequate support to their caregivers.

Description of the project:

Acehealth is about developing and implementing telemedicine to offer comprehensive care to people with cognitive impairment or dementia. It will be necessary, among others, to develop a platform that will collect the fundamental aspects of patient care: diagnostic visits, the follow-up and the individualized therapeutic intervention plan.

The specific objectives of the project are to minimize trips of medical visits in order to avoid risks of contagion, bring the diagnosis closer to people who are in rural areas or at a distance, facilitate early detection of people who are concerned about their memory, saving time and other visits and lastly, to facilitate the caregivers in reconciling and caring for their family members with their work and personal life.

With Acehealth, patients and caregivers will have a contact tool with a multidisciplinary team with more than 25 years of experience in the diagnosis, research and treatment of dementia. Likewise, the implementation of others technologies will be necessary to carry out the project, for instance, a virtual switchboard. Specific training will also be required for all professionals.

People with Alzheimer’s or dementia and their caregivers.

2,000 a year.

The scope of the project in relation to the number of beneficiaries and the problem it covers:

In a situation of restricted mobility due to the pandemic and in case of patients with mobility difficulties or who resides in remote places. Acehealth will be able to offer specialized medical care to patients and their families.
